ApplicationsNew ApplicationsThe applicant, regardless of age, pays a fee of $350.00 for lifetime membership. Checcks should be payable to the "OFNA" for $300.00, and to "OFNA Genealogist" for $50.00. These fees includes a comprehensive review by the Genealogist and administrative costs. The Membership Inquiry Form is available under DOCUMENTS on this web site If approved, the fee paid becomes a lifetime membership payment; meaning no other monies will ever be due — except for future supplemental applications. Supplemental ApplicationsCurrent members may submit supplemental applications. The fee for supplemental applications is $150.00.
